To redeem your promotion for cases with physical impressions, package your promotional Rx or promotion certificate with your impression. Then, schedule a shipping pickup by calling us at 800-854-7256 or by logging into myaccount.glidewelldental.com.
To redeem your promotion for cases with digital impressions, type the promotion code in the comments or notes section of your scanner’s digital prescription form. Or, if submitting the case digitally through My Account, type the promotion code in the text field within the Send a Digital Rx form section on myaccount.glidewelldental.com.
Promotions are not redeemable for cash and cannot be combined with any other offers, including volume pricing. Not valid on prior purchases. Not valid for customers with enterprise or contract pricing. Promotions must be included at the time of case submission to apply.

Promotions are subject to case approval and receipt of all required records (e.g., Rx, impressions, photos, etc.).
