Being the Director of Clinical Education at Glidewell Laboratories, I have the opportunity to see crown preparations from thousands of dentists all over the country on a weekly basis. The biggest problem that I see is lack of reduction, which leads to bulky, unesthetic crowns. These crowns can also wreak havoc with the gingival tissues if they are over-contoured in the gingival third. As a result, I came up with a preparation technique based on depth cuts that ensures adequate reduction every time. Fortunately, this preparation technique does not take any longer than a standard technique. I worked with Axis Dental to put together a bur kit with the least amount of burs to accomplish this technique, called the Reverse Preparation Technique. It’s called the Reverse Preparation Technique because the preparation begins with preparing the gingival margin first. This is the opposite of how most of us were taught to prepare teeth in dental school. From looking at many models of crown preparations, it is apparent that preparing a consistent gingival margin is difficult to accomplish on a regular basis. By using a round diamond to prepare the gingival margin first, you ensure that you prepare an almost ideal gingival margin.
