The secret to preventing patient gagging is a solution called DYC rinse (formerly Cyclone DS rinse), which is a 1% dyclonine preparation available from Steven’s Pharmacy. Ask the patient to swish with a mouthful of DYC rinse for 45 to 60 seconds, and all the soft tissue it touches is anesthetized. I tell patients they will not gag if they use the DYC rinse and so far I have been 100% correct. On the hygiene side, my hygienist uses DYC rinse for periodontal probing for patients with inflammation and gross debridement patients who are sensitive. We see a lot of patients who are overdue for hygiene but do not need scaling and root planing. Because we typically only use local anesthetic for root planing patients, DYC rinse helps fill the gap as an easy-to-use topical anesthetic that provides peace of mind to the patient.
